Job Description

A community-based non-profit in Manhattan is seeking a Licensed Clinical Social Worker. The LCSW will be responsible for performing a thorough psychosocial assessment for each patient enrolled in the stroke study during the initial home visit within 72 hours of discharge. Based on this assessment, the LCSW will create a plan to provide one hour of individualized short-term counseling per week over a three-month period.

The base pay range for this position is $35 to $35.75 per hour. The actual compensation offered to the successful candidate may vary from posted hiring range based on geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of the offer. This is a per diem assignment!

Responsibilities:

• Perform thorough psychosocial assessments.
• Offer therapy and psychotherapy to residents or refer them to external services when necessary.
• Implement interventions and evidence-based treatments.
• Supervise case management activities.
• Offer emotional support, guidance, and necessary resources to help patients address and resolve their issues.
• Engage in active case management and follow up with patients.

Requirements:

• Bilingual in Spanish and English
• Master’s degree in social work
• Active NYS LCSW License
• 2-3 years of experience working with low-income populations
